These terms apply to care.diagnexhealth.com, the public patient and family information and enquiry website operated by Carehorizon Health Private Limited under the Diagnex brand.
Diagnex Care helps patients and families understand care pathways, locate relevant information and submit basic enquiries. The website is not itself a treatment plan, appointment confirmation, clinical record or guarantee that a service is available.
Website information is general and does not replace patient-specific medical advice. Diagnex Care Managers do not diagnose, prescribe, alter medication or replace the treating doctor. Where a pathway depends on clinical eligibility, prescription or professional assessment, that requirement remains in force.
Diagnex-wide cities include Mumbai, Navi Mumbai, Kolkata and Howrah. Individual services are not automatically available in every locality. Scope, staffing, equipment, current capacity, timing and any clinical dependencies must be confirmed for the relevant request before a commitment is made.
Pages describing Advanced Care at Home do not represent hospital-equivalent or ICU-equivalent capability.
Submitting a website form or requesting a date does not create a confirmed appointment or service contract. Diagnex will review the enquiry and explain what needs to be confirmed. Do not submit medical reports, prescriptions or detailed clinical histories through the ordinary website form unless Diagnex specifically directs you to an approved protected workflow.
For an emergency, use the appropriate local emergency service or nearest suitable medical facility.
The Care website does not currently collect card or banking credentials. If a Diagnex service requires payment, the applicable invoice, payment route and any service-specific terms should be provided through the relevant approved Diagnex billing or payment workflow. Never send CVV, PIN or OTP through a website enquiry or email.
Diagnex website content, branding, layouts and materials are intended for lawful personal information use. Unless otherwise permitted, users should not copy, misrepresent, resell or use Diagnex branding in a way that suggests an unauthorised relationship.
The Care website may link to other Diagnex websites or third-party services. A separate website may have its own privacy notice, terms or security controls. Users should review the terms that apply to the destination being used.
Diagnex may update these terms when the website, care pathways, legal obligations or operating practices change. The effective date above identifies the current published version.
