How Diagnex Care handles personal information.
This Privacy Policy explains how Carehorizon Health Private Limited, operating under the brand Diagnex, handles information submitted through care.diagnexhealth.com and its patient and family enquiry workflows.
1. Scope and responsibility
Carehorizon Health Private Limited ("Carehorizon", "Diagnex", "we", "us" or "our") is responsible for personal information processed through the Diagnex Care website and the public enquiry workflows described here. This policy does not replace a more specific privacy notice that may apply to a separate patient application, clinical record system, doctor portal, partner portal or other protected Diagnex workflow.
Brand: Diagnex
Email: care@diagnexhealth.com
Phone: +91 62920 41333
2. Information we collect
The public Care website is designed to collect only the information needed to contact and route an enquiry. This may include your name, mobile number, city, broad care need, preferred contact time, enquiry source and limited campaign/referral information where present.
We may also process communications created after an enquiry, such as follow-up messages, scheduling information or a grievance. We do not ask for full card details, CVV, PINs or banking OTPs through Care website forms.
3. How we use information
- to respond to a patient or family enquiry;
- to identify the relevant care pathway or team;
- to confirm locality, scope and operational availability before commitment;
- to coordinate requested services if you proceed beyond an enquiry;
- to maintain website security, prevent abuse and keep auditable operational records;
- to address complaints, privacy requests and service-coordination concerns;
- to comply with applicable legal, regulatory, tax, accounting and contractual requirements.
4. Health and clinical information
The public website is an information and enquiry channel, not a clinical record system. If a care pathway requires health information, Diagnex should direct you to the relevant approved protected workflow and limit access to people who need it for an authorised purpose.
6. Retention
Information is retained only for as long as reasonably necessary for enquiry follow-up, service operations, audit, security, legal claims and applicable statutory requirements. Specific retention periods depend on the system, record type and applicable legal or operational requirement. Information that is no longer required should be deleted, anonymised or securely archived according to the relevant policy.
7. Security
Diagnex uses administrative and technical safeguards appropriate to the system and information involved. These may include HTTPS, secure sessions, access controls, server-side form handling, restricted storage and audit records. No internet service can guarantee absolute security, so users should avoid sending unnecessary sensitive information through ordinary email or website forms.
8. Your choices and requests
Subject to applicable law and the context of the request, you may contact Diagnex to ask about information associated with a website enquiry, request correction of inaccurate contact information, request deletion where information is no longer required and no lawful retention obligation applies, withdraw consent to future contact where consent is the basis for that contact, or raise a grievance about how information has been handled.
We may need to verify identity before acting on a request.
10. Children
The public Care website is not designed for children to independently submit healthcare information. A parent, lawful guardian or authorised adult should use Diagnex channels on behalf of a child where appropriate. Child-related clinical information should be handled only through the relevant approved care workflow.
11. Privacy and grievance contact
Email care@diagnexhealth.com, call +91 62920 41333, or use the Grievance & Escalation page. Please provide only the minimum information needed to locate the relevant enquiry.
