Preparing for home care.

Before the first visit

• Keep recent prescriptions, discharge summaries and relevant reports available.
• Confirm the patient’s address, access instructions and preferred contact person.
• Prepare a clean, well-lit area appropriate to the service.
• Share mobility, infection, access or safety concerns in advance.
• Ask which medicines, consumables or equipment must be available.

For advanced or equipment-supported care

Additional assessment may be required for power, water, space, ventilation, caregiver support, emergency access and transfer planning.
